Our experienced mentors provide personalized guidance for individuals seeking help to navigate the complexities of accommodations on campus, finding resources, and/or connecting to Mental Health Services. 2. IncludEd SafeDose Project
Our Free Medication Lock Box Program is designed to provide young adults in Utah who are aged 18-24 with access to secure and convenient storage solutions for their prescription medications. This program aims to promote safe medication management practices and reduce the risk of misuse among this demographic.
Qualifying participants will receive a free medication lock box upon request, offering a simple yet effective way to store and protect their medications. The lock boxes are designed to be easy to use and portable, making them ideal for young adults who may be living independently or sharing living spaces with others.
Once you fill out this form, an individual from IncludEd United will contact you from your selected communication preference (i.e., text, email) to let you know the expected time frame and delivery or pick-up method for your medication box.
The information gathered in this form are for grant reporting purposes and to get the medication boxes to the participants. This information will not include any personal identifying factors (name/address/phone number/ email address/etc) in grant reporting.
